how do i beat the crystal sage?

I'm level 45 using the vordt great hammer and have a poise of like 18. I keep getting demolished by the sage and once she summons her mimics their spells just keep destroying me before i can get close to the real sage and attack. and once i attack she switches to her melee and obliterates me. How do i kill this bitch?
