PSLF Administrative Forbearance - how to request?

I just submitted my final ECF electronically last week and would like to go on forbearance while I wait for forgiveness. Because I consolidated back in 2016, FSA wouldn't let me check the box that says "I qualify for forgiveness now, put me in forbearance while I wait".

I called MOHELA today and got the run around. Waited 30 minutes for a call back and when they called me back, it hung up after a few seconds. Called back and waited another 30 minutes for a rep, who said that she would have to transfer me to "Advantage" (maybe Aidvantage?) to request a forbearance. She transferred me and it rang multiple times, but then it seemed to return to MOHELA's system and said I'd have to wait for 50 more minutes to speak to someone.

At this point, I gave up and decided I'd deal with it later. Anyone know if I can just fill out the General Forbearance Request form and upload to MOHELA online? My payment count is currently at 115, but should go to 120 once my most current ECF is processed. I've seen some posts saying they won't grant you forbearance until your payment count actually says 120.
