Is anathema a separation from the Church pr call to repentance?

I see anathemas directed towards heretics and heterodox. Does that mean that they have to ask for permission from the Church to be Orthodox and does that mean that if they don't return to Orthodoxy they will be condemned to hell or is anathema just a call to repentance?
