Help with a pill mill

Hey all - I’m having somewhat of a rough morning. I didn’t expect to be dealing with a midlevel getting to my family, but here we are and so I am struggling with next steps.

I had a family member prescribed ketamine listed under a PA. They have only seen this person in virtual and were instructed to take this at home. They are fortunately okay now, but were found obtunded. I’m a surgeon and have never had to file a report against another persons license, but that is where I’m going to start. The bigger issue is that this person seems to be under a larger virtual care agency and I strongly suspect that they are just throwing narcotics out right and left

How do I go about not only addressing the person who prescribed this, but also the agency who is supervising and likely encouraging this kind of behavior?
