Anybody know what could cause this? (Drywall)

It doesn’t seem to be water damage, there are no bubbles and the drywall isn’t soft or wet when it appeared - but it is under my upstairs shower.

I live in a 100+ year old house so not surprised by these things anymore but trying to figure out what could cause this and how big of an issue it is.
